Guidelines for Planning Transition

  • Include the student and family in transition planning.
  • Provide student with options to participate in multiple community events and experiences.
  • Include vocational evaluation results in transition plan after age 16.
  • Develop a multiple-year plan to fashion the transition over time.
  • Plan for transitions in home, school, work and community.
  • Use quality of life indicators to assist in planning and evaluating transition goals.
