Guidelines for Planning Transition
- Include the student and family in transition planning.
- Provide student with options to participate in multiple community events and experiences.
- Include vocational evaluation results in transition plan after age 16.
- Develop a multiple-year plan to fashion the transition over time.
- Plan for transitions in home, school, work and community.
- Use quality of life indicators to assist in planning and evaluating transition goals.
